PLEASE INFORM YOUR AESTHETICIAN BEFORE YOUR SERVICE
Your health and safety are important to us.
Before receiving eyebrow threading, eyebrow or facial waxing, body waxing, eyebrow tinting, or other beauty services, you must tell your aesthetician before the service begins if you have any known or suspected allergy, sensitivity, or prior reaction to hair dyes, eyebrow or eyelash tints, waxes, cosmetics, adhesives, fragrances, latex, skincare products, or any other product or ingredient that may be used during your service.
- Have any known allergies or sensitivities to cosmetics, dyes, tinting products, wax, adhesives, fragrances, latex, skincare products, or other ingredients.
- Have previously experienced an allergic reaction, irritation, swelling, rash, burning, or other adverse reaction following a beauty or cosmetic treatment.
- Are currently using prescription or over-the-counter medications or topical skincare products that may cause increased skin sensitivity.
- Have broken, irritated, sunburned, inflamed, recently treated, or unusually sensitive skin in the area being serviced.
- Have any other condition or concern that may affect your ability to safely receive the requested service.
ALLERGIC REACTIONS & SKIN SENSITIVITY
Beauty and cosmetic products may cause an allergic reaction or skin irritation in some individuals, even if you have used the same or similar products previously without a reaction.
If you have a known allergy, sensitivity, previous reaction, or are unsure whether a product is appropriate for you, PLEASE INFORM YOUR AESTHETICIAN BEFORE YOUR SERVICE BEGINS.
When appropriate, your aesthetician may recommend a patch/sensitivity test, modification of the service, or postponement of treatment.
For your safety, we reserve the right to decline or discontinue a service when there is a reasonable concern that the treatment could cause injury, irritation, or an adverse reaction.
EYEBROW TINTING NOTICE
Eyebrow tinting products must be used in accordance with applicable federal and California requirements and the manufacturer’s instructions.
Please immediately tell your aesthetician if you experience burning, excessive stinging, itching, swelling, pain, unusual discomfort, or irritation during any service.
ACKNOWLEDGMENT & ASSUMPTION OF INHERENT RISKS
By choosing to receive a service, you acknowledge that threading, waxing, tinting, and other cosmetic services can involve inherent risks, including temporary redness, irritation, skin lifting, sensitivity, swelling, or allergic reaction. You agree to promptly disclose known allergies, sensitivities, medications, topical products, recent skin treatments, and prior adverse reactions that could affect the service.
To the fullest extent permitted by California law, you assume the inherent risks of the service and are responsible for losses or expenses resulting from an inherent risk or from information you failed to disclose, including medical expenses or lost wages. Nothing in this notice releases or limits liability that cannot lawfully be waived, including liability arising from fraud, willful misconduct, violation of law, or other liability that California law does not permit to be released.
